Affidavit of Correction (T-11)

A T-11 is used to explain allowable corrections in an assignment section on the back of the title or statement of origin. It must be completed by the seller, and the seller's signature must be notarized. Examples of when to use the form include if the seller signs in the incorrect space, or a buyer decides not to continue with the transaction and the seller sells it to someone else. One thin line can be drawn through the incorrect information. It cannot be used for titles where white-out, correction tape or complete scratching-out of the information has occurred.

Affidavit of Inheritance (T-20)

This form is completed by an individual who has inherited a motor vehicle where there is no will or a will with limited assets.

Application for Replacement Plate and/or Decal (MV-7)

This form is used by tag office associates to provide a replacement plate/decal. If your plate was damaged or lost in the mail, simply visit a tag office and the associate will complete this form for you. However, if your plate was otherwise lost or stolen, you must first report it to the law enforcement where the incident occurred or, if unknown, the law enforcement of your home address and obtain a copy of the police report. You must present a copy of the police report at the tag office to request a replacement plate or decal for lost or stolen plates. Tag office associates will complete the MV-7 at that time.

Affidavit to Certify Immediate Family Relationship (MV-16)

This affidavit provides evidence that the listed vehicle was transferred (whether gifted, purchased or inherited) between persons who are immediate family members (spouse, parent, child, sibling, grandparent or grandchild). All owners must have the immediate relationship. A form for each transferor to each transferee is required when transferring ownership.

Bill of Sale (T-7)

This form can be used to transfer ownership of personal property. It can be used in lieu of a title on motor vehicles with model years 1985 and older where there is no title and trailers that are not titled in Georgia (weighing less than 2,000 lbs).

This form is used for inspection of non-titled vehicles, salvage vehicles and for completing a title bond.

Extension of Dealer Tag (T-226)

This form is used when requesting an extension because a dealer is still processing the title paperwork on the buyer's behalf. The form can be completed and brought into a tag office where the buyer resides along with the dealer drive-out tag, bill of sale, and proof of insurance. An extension can be obtained for up to 30 additional days if the buyer comes in within five days of the expiration date on the dealer drive-out tag.

Insurance Fee Waiver Form (MV-18J)

The registered owner of a vehicle may apply for a one-time waiver of the pending-suspension and/or the reinstatement fee if certain criteria are met. To apply for this waiver, the registered owner of the vehicle must complete an Insurance Fee Waiver Form.

Lien Release (T-4)

If you're applying for a duplicate title and there is a security interest still shown on the vehicle record, this form can be used to release that interest when completed by the lienholder. All signatures must be original; faxed or emailed signatures are not accepted.

Multipurpose Off-Highway Vehicle Inspection Affidavit (MV-OHV)

This affidavit is to be used to obtain a certificate of registration and license plate for a multipurpose off-highway vehicle.

Prestige Application (MV-9B)

This form is completed when requesting the manufacture of a prestige license plate for a motor vehicle or motorcycle. There is a designated area to list first, second and third choices for the plate.

This form is to be used to apply for a military license plate.

Senior Emission Waiver

If the registered owner of a motor vehicle is 65 years or older, and the vehicle to be renewed or registered is at least 10 years old and is driven less than 5,000 miles per year, this application can be completed to exempt the emissions for that vehicle. The vehicle must have a functional odometer, and a new application needs to be completed each year with the current mileage on the vehicle. Apply online at the Georgia Clean Air Force website.

Title Application (MV-1)

This form is used to apply for a Georgia title and should be completed by the buyer of a motor vehicle, trailer (over 2,000 lbs) or mobile home. Download the form below or begin the process online on the Department of Revenue's e-Services site and save time in the tag office.

Title Bond Paperwork

A title bond can be used when a title cannot be located or corrected on vehicles 1986 and newer. Title bond companies are used to secure a title, and the must complete the following forms:

Voluntary Cancellation
